Please read the following statements and indicate below whether you have read and understood the details and would like to participate in the survey.
 
1. I confirm that I have read and have understood the information sheet dated 8th December 2023 for the above study. I have had the opportunity to consider the information, ask questions and have had these answered satisfactorily.

2. I understand that taking part in the study involves completing a short online questionnaire about my experience of the pilot programme, expectations about the service, and some general demographic information.

3. I understand that my participation is voluntary and that I am free to stop taking part and can withdraw from the study at any time without giving any reason and without my rights being affected. In addition, I understand that I am free to decline to answer any particular question or questions.

4. I understand that the information I provide will be held securely and in line with data protection requirements at the University of Liverpool until it is fully anonymised and then deposited in the archive for sharing and use by other authorised researchers to support other research in the future.

5. I understand that if I voluntarily choose to enter the prize draw competition, I will be providing my email address, and this will be stored securely on University of Liverpool servers separately from my survey responses. If I have submitted my email for the prize draw, I understand as soon as the winners of each draw have accepted their prizes my email address data will be destroyed. Only the research team will have access to my email, and this will not be included or associated with any future presentation or publication of the questionnaire data.
